Patients Wellness & Education Center
New Passion Home Care is very focused and dedicated to our patients, patient’s families and caregivers. One of the vital parts of home health care is disease and medication teaching to our patients and their caregivers. Our professional and passionate clinical team works very close to teach those diseases and medications risk factors etc., during our care; however, we are very concerned with the potential forgetters of such teaching. Therefore, we are committed to keep our patients, caregivers, and their family members informed and educated as much as possible with general public knowledge, even after being discharge from our professional and passionate home health care services. You may always reference to our site for common public and general health care information to always keep you educated on good health and wellness.
Patients and caregivers education encourage, remind, and attract patients to comply with their plans of care, which can improve outcomes. It helps patients become more compliance. Noncompliance patients include patients’ failure to take prescribed medications regularly, or to finish or renew prescriptions, or not eat healthy, or not perform exercise programs, and the list will go on and on. While compliance may induce patients to take over prescribed medications they don’t need, patients who comply with high-quality care plans will obviously benefit.
We recognize health and wellness education is important in fostering the motivation, skills and confidence necessary to take action to improve health overall. Our health and wellness educationincludes the communication of information concerning the underlying social, economic and environmental conditions impacting on health, as well as individual risk factors and risk behaviors, and use of the health tips.
We believe in increasing our patients, families, and caregiver’s knowledge on healthy lifestyles and the importance of compliance in delivering our passionate and quality care. There is substantial evidence that patient healthcare education and wellness programs add much to people’s health and well-being.We hope and to keep you educated as much as possible on wellness and good health practices to hopefully help you avoid illness and stay educated on your health conditions.
Research has shown Patient as well as Patient’s Caregiver education on health and wellness as well as understanding illness and/or diseases will directly influence behavior changes needed to maintain and improve health through changes in knowledge, attitudes and skills in care. It requires more than simply giving patients written instructions, a pamphlet to read, lists of resources, or a video to watch; effective patient education includes discussion, demonstration, and active participation, in which we practice well in our passionate quality care and education during care to our patients. However, we also understand how patients and caregivers forget at times, therefore, we encourage you to stay educated by visiting those materials and refreshing the teaching we conducted.
Benefits of patients and caregiver education include but not limited to:
