While Undergoing Our “Specialized Clinical Programs” & “Quality Passionate Care”:
We have customized and compiled structured patient education materials that provide “Need-to- Know” information in a very low grade literacy format and easy to read by way over sized letters and spacing. New Passion Home Health Care took this initiative as part of our spirit to promote Care Transitions Programs in the effort to prevent acute care hospitalization or re-hospitalization, and improve patients’ quality of life and as much as possible independence as their conditions permit. Patient proper education and interventions by the home health care will be the key components to keep the patient living quality life and avoiding unnecessary visits to emergency room and utilization of emergency acute services and/or facilities.
Our model is to provide our patients with specific education and training components and material that are most appropriate to the care needed in correlation with diagnosis, Plan of treatment, and quality healthcare services we provide to them. Those are specific New Passion Step-By-Step Patient Education Programs and Spotlight Zones Red Flags/Alerts!
These Care Transitions Programs are designed in response to the need for a patient-centered, home medical model, interdisciplinary intervention that addresses continuity of care across multiple settings and practitioners. The overriding goal of the intervention is to improve quality care transitions by providing patients with Education Programs and Transitioning Tools/Support that promote knowledge and self-management of their condition as they move from hospital to home. A pillar of this program is patient knowledge of “Red-Flags” and “Alerts” that indicate disease exacerbation that could lead to Acute Care Hospitalization when it’s necessary.

Patient Stoplight Clinical Program
This Patient Stoplight Clinical Program was designed in response to the need for a patient-centered and interdisciplinary intervention that addresses continuity of care across multiple settings and healthcare practitioners. Our ultimate goal is to improve care transition by providing patients with tools and support that promote knowledge and self-management of their condition as they move from hospital to home. This Stoplight tool is to be integrated and launched with the rest of the primary and secondary Patient Education Programs. A pillar of this Patient Education Program is the patient knowledge of "Red Flags" that indicate disease exacerbation that could lead to Acute Care Hospitalization. Therefore, our goal is to educate and provide tools to the patient and their caregivers so unnecessary re-hospitalization will not occur.
